Name and Particulars: Ensure that the information entered in the form exactly matches the details provided to the University/College.

DOCUMENTS REQUIRED FOR REGISTRATION:

All original certificates and address proof must be provided in A4 size color Xerox only.
Follow these steps:
a) Print the application form in color on A4 size paper, single-sided.
b) Submit the following documents:
Provisional Certificate (2nd copy) issued by the University after completion of CRRI.
Compulsory Rotatory Residential Internship (CRRI) Certificate.
Transfer Certificate (TC).
Aadhaar Card as address proof (valid for Puducherry addresses only).
Two recent passport-size photos for Registration.

REGISTRATION DELAYED BY MORE THAN SIX MONTHS:

If arriving after six months or more, provide:
a) Degree Certificate issued by the University.
b) Original affidavit issued by a Notary Public explaining the delay in registration. Download Affidavit format here.

BDS REGISTRATION WITH NOC FROM OTHER STATES:

For BDS registrations with No Objection Certificate (NOC) received from other states:
a) Degree Certificate issued by the University.
b) Original No Objection Certificate (NOC) from the respective State Dental Council and Dental Council of India issued within the last 6 months.

FEE PARTICULARS:

Payment of Rs. 3000/- should be made through NEFT/Google Pay/PhonePe.

b) Surrender the Original Puducherry Dental Council Registration Certificate for cancellation.
c) Provisional Certificate / Degree Certificate issued by the University.
d) Course Completion Certificate / Transfer Certificate issued by the College.
e) Aadhaar Card as Address proof (valid for Puducherry addresses only).
f) Submit two recent Passport size Photos at the time of Registration. (Note: Do not provide the same photo submitted during BDS Registration).

c) Course Completion Certificate (issued by the Dental College).

f) Mark Sheet - First year to Last year should be taken on separate pages (issued by the College).
g) Two Recent Passport size Photos for Registration.
MORE THAN SIX MONTHS DELAYED REGISTRATION:

If a candidate arrives after Six months or more, they must provide an affidavit issued by a Notary Public for the delay of registration. (Original affidavit should be submitted at the time of registration). [Download Affidavit format here.]
FOR BDS REGISTRATION WITH NOC RECEIVED FROM OTHER STATES:

No Objection Certificate received from the respective State Dental Council and Dental Council of India within the last 6 months should be submitted (Both the Original NOCs).

b) In case of a misplaced Puducherry Dental Council Registration Certificate, include:

FIR with registration number and other relevant details.

Registration Certificate issued by Puducherry Dental Council.
c) Aadhaar Card as Address proof (valid for Puducherry addresses only).
d) Two recent Passport size Photos for Registration.
e) If a Candidate cannot appear in person:
Fill the Form duly signed and provide a separate authorization letter.

c) Address proof - Aadhaar Card is Mandatory (Tamil Nadu Address only).

APPLYING FOR EXPIRED NO OBJECTION CERTIFICATE:

a) Surrender the Original Expired No Objection Certificate for Cancellation at registration.
b) Provide a handwritten letter from the applicant stating the reason for the expiry of the No Objection Certificate. [Download Expired Letter format here]

APPLYING FOR DUPLICATE NO OBJECTION CERTIFICATE:

a) Provide an FIR for the misplaced Tamil Nadu Dental Council Registration Certificate, mentioning the registration number and other details.
b) Submit the Original Affidavit from a Notary Public for the duplicate No Objection Certificate. [Download Affidavit format here]
